What To Expect At Your First Visit
WELCOME!! We are very pleased to meet you!
We want your office visit to be as comfortable and pleasant as possible. Please arrive 10 minutes prior to your scheduled appointment to allow adequate check in time. For your convenience, we mail out a welcome packet in advance for you to fill out. If you prefer you may download and fill out the New Patient Forms. (These forms can be filled out on your computer then print the completed form to bring in. You will need Adobe Acrobat Reader)
If you would like us to obtain your previous records, please contact the office you will be seen in or download the necessary form here.
During your visit the doctor will dilate your eyes.

If you wish to waive this recommended portion of your exam, please notify our office when scheduling your appointment. You may download the required waiver or fill one out in the office at the time of your exam.
Our Privacy Policy is posted in our office and is available for viewing online (it can be downloaded and printed at your convenience). A copy of our Privacy Policy can also be supplied to you in our office upon your request.
